Dam was constructed under supervision of Cushing & DeWitt

 Details;  Length= 475',  Height= 20',

Hgt above level of full pond= 5',   Avg depth of water= 10',

Length of rollway= 25'

West Dike: Length= 310',  Height= 10',  Width= 25'

> 150 years old

1886   Stated in 1933 report that the   "...Feb 12-15, 1886 storm resulted in the water level rising by 6 feet in that brief period."  
1868 1920 Ten events recorded with level over 4" above the old spillway >  11.5' lake level
1924   Ordered that water level to be kept low until repairs are made  
1927   Report states that water level has been kept low for past four years and that a lower level was ordered until repairs were made.  Owner agreed to keep water level to 7 feet below the spillway ~ 4' lake level
1928 Dec Ordered that water level to be kept at least 5.5' below the spillway.  Also, a gauge was ordered to be placed at the gate house. ~ 5.5' lake level
1929 May Ordered to comply with 5.5' below spillway prior order.    
1929   Request made "…so that a level of about 9' (or 3' below the spillway) be maintained during the summer months instead of the the 5' or 6' that now prevails…" ~ 8' lake level
1933   Records which start with 1867 through 1928 indicate that the reservoir was drawn upon each year to its entire capacity and that by August or September of most years it was completely exhausted.  
1934   First time written rules for water level during different periods of the year.     For the April-Aug period, the max allowed level was set at 3' below the spillway. ~ 8' lake level
1938 Sept

One of the worst hurricanes to hit Rhode Island.   Local resident remembers seeing the lake level being 1' or  2' below the floor of the bridge

1939   Repairs were made to the control structure repair
1943   Report suggests that level be kept about 4' below the spillway  
1976   Constructed a new bridge across the spillway bridge
1983   Trash rack was replaced, cleared brush & trees, & repaired rip-rap repair
1985   Built cinder block house & added fencing  
1992 July Constructed a new bridge across the spillway (steel) bridge
1992   Smithfield Fence replaced the fence at the gate house repair
1993 May Brush clearing party brush
1993 June Shaft was pulled and repairs were made to the gate structure repair
1993 June Replaced roof on gate house $950.  Wind blew old roof off repair
1993 June $250 appropriated for diver to make repairs under water repair
1994 Nov Volunteers cut brush on lake side of the dam brush
1995 Nov Volunteers cut brush on dam brush
1997 July Some work on spillway (no details)  
1998 Jul-Oct Court Order giving Pascoag Upper Dam Association control of water level & access to dam house (PUDA authority was passed on to the new Pascoag Reservoir Dam Management District).  
1999 June Idea for a special tax district was discussed at annual meeting

2012 Oct New roof & painted gate house (Rylah $425)

2013 July Emergency repair of broken gate stem.   Work was done by InnerTech.  Labor= $4,313, Material= $1,150   Total= $6,463

  Had a financial audit done - ($700)  
2014 Oct Added a guide above the gate to keep it squared up  (Innetech $2,174)  

2015

Mar Had Innertech inspect new guide - concern it may had come loose... Finding all was well  

2015

Aug Hired Innertech to install sacrificial anode -  ($544)  
2015 Aug Vegetation control treatment by VCS  ($475)
